Care planning to improve retention

Abstract
Coventry CDT implemented a comprehensive review of its care planning system as part of its performance review to improve retention. The service had been performing poorly when measured against other services and was under scrutiny from its commissioners. Prior to this significant improvements to service redesign had resulted in the elimination of waiting times to access the service for a comprehensive assessment. Following assessment a significant of clients failed to access treatment.
In an attempt to improve retention an ‘initial care plan’ was adopted, whereby clients received a document outlining the outcome of the assessment and services on offer. An important part of the document was a checklist of services agreed with dates for first appointments.
Following piloting this with clients it has been included in the assessment and information pack that clients receive as a tailored piece of information directly relevant to them. Since implementing the ‘initial care plan’ there has been a significant improvement in retention, both in accessing the service and staying in treatment.
